Maternal Health Manager

Horizon Health Care is seeking a full-time Maternal Health Manager to assist with development and implementation of program infrastructure including policies and procedures relevant to the program.
Assist with daily tasks, including but not limited to, communication related to the program, ensuring grant metrics are achieved, and collaborating with other Horizon Health Care leaders and medical providers to ensure program success.
Successful candidates for the position should be passionate about providing high-quality, patient centered care.
Horizon Health Care offers a team-oriented work environment focused on meeting the needs of individual patients while supporting and contributing to the health and wellness of our communities.
This position can be located in any Horizon Health Care location.
Remote would be considered for the right candidate, with travel required.
